Annual Reports for Guardianships and/or Estates


Annual Return
This form is used to report to the Court all receipts and disbursements for a given time frame associated with the appointment of the guardians and/or executor or administrator.

Inventory Sheets
This form is used to report to the Court all the inventory of the estate, both real and personal.

Personal Status Report
This form is used to report to the Court the status of a ward (minor or adult). This report is filed each year by the Guardian.

Asset Management Plan and Inventory
This form is required within two month of appointment of a Conservator. This is so the court has a plan for managing, expending and distributing of the property.
